- We will have no mental health provider. If you were scheduled after this date please contact the office at 573-803-3995 and we will help you transition into a new provider.

Appointment Times
Please be aware of your scheduled appointment time. If you cannot make your appointment time please contact the office as soon as possible.
Appointment reminders will go out to the phone number/email that we have on your chart. If you are not getting your reminders please check that we have the correct information for you and that you have not opted out.
Deductible, Co-Pay and Co-Ins
ALL DEDUCTIBLE AMOUNTS, CO-PAYS, AND CO-INS ARE THE RESPONSIBILITY OF THE PATIENT AND WILL BE REQUIRED TO BE PAID IN FULL.
Please try to get these paid as soon as possible. We will do our best to work with you.
Self Pay Prices
We strive to keep our self pay prices as low as possible. Unfortunately we have to charge what CMS requires or we can be in breech of contract with the insurances. We do require payment prior to any scheduled appointment or any cosmetic procedure. Please contact the office at 573-803-3995 for any questions.
