Our weight loss clinic requires an evaluation prior to treatment. Prior to the evaluation we require the patient forms to be filled out and submitted to us so that they can be reviewed. We are offering a few different options for the weight loss. 

One option will be a weekly shot and price will be dependent on how many milligrams you will be on. The initial visit (evaluation and will include the shot) will cost $110*. With this option you can pay ($60*-$100* is depending on your milligrams)by the week at your check-up (check-in with the nurse and shot is included) or you can pay by the month. 

Another option will be an evaluation with medications sent to your pharmacy of choice. This option will cost $110* and will include check-ups with the nurse during the month (these can be telehealth). We are offering a couple of different medications for this option.
